Our Memory Support Neighborhood offers a gentle and secure environment for individuals with memory loss and Alzheimer’s disease. Residents’ needs are addressed with compassion by staff professionally trained in Alzheimer’s and dementia care.

Our goals are to enhance residents’ existing strengths and skills and to minimize limitations through “failure-free” therapeutic activities and services. Daily activities are based on the concept of creating pleasant days. Each resident’s Wellness Care Plan is developed in cooperation with family and reflects the resident’s interests, hobbies and professional skills before the onset of the impairment. This approach, with strong family involvement, helps residents maintain their individuality, independence and dignity.

Our innovative programming, expert compassionate staff and homelike environment offer individuals with Alzheimer’s the best quality of life and their family’s peace of mind. We recognize that the effects of Alzheimer’s disease are far-reaching. We encourage family involvement in developing their loved one’s Wellness Care Plan and we provide on-going services to family members including regular conferences, support group meetings, educational programs and a resource library.

Concord Park is a monthly rental community offering Assisted Living and state-of-the-art Memory Support Care. Our supportive services, delivered with care and compassion, enhance residents' independence and dignity and provide peace of mind for residents and their families. We offer a range of accomodations and service packages. Specific monthly rates depend upon the size and location of the apartment, as well as service options chosen by the resident. Follow I-95/Rt. 128 to Rt. 2 West for approximately 7 miles. At second light past Emerson Hospital, take a left onto Rt. 62 West toward West Concord.
