When someone joins the Cornerstone at Milford community, we become their partner in healthcare.
Along with the wellness and support services that residents have access to at Cornerstone, we communicate with your healthcare providers to maintain a deep understanding of your care needs. For those seeking healthcare providers, we collaborate with a variety of local practices who bring their services directly to the community or are located in the greater Milford area. Whether you are already a resident at Cornerstone or are transitioning to your new home, we have your back. From in-house primary care services to strong partnerships with local care providers like PT/OT, vision, dental, mental health counselors, we are truly a hub of resident care.
Our approach is centered around resident choice. How you build your healthcare provider network is completely up to you. When you make the move to Cornerstone, you have the option of working with the providers you feel more comfortable with and continuing those connections, or you can work with our onsite partners.
Onsite Care Partners
Primary Care Coordination: Visiting Physicians, GCMs, Lab Services
We work with Vantage Healthcare to offer onsite primary care, bringing health professionals right to your apartment. Vantage’s team includes an onsite Nurse Practitioner (NP) and a remote Nurse Care Coordinator (RN). Benefits of this partnership include:
- Primary care services onsite once a week at the community
- Services are often covered by Medicare
- Coordination of other health care services such as specialists, PT, OT and speech therapy providers.
Residents seeking a Geriatric Care Coordinator have the option to work with Aging Well Advocates and The Aging Space, who provide onsite eldercare consulting, recommend care options, and offer emotional and family support as needed.
We partner with Quest Diagnostics for lab draws.
Mental Health & Wellness Services
If you need extra support to work through challenges and changes in life, our partnership with Vantage Healthcare ensures that you are not alone. With this partnership, residents have:
- Access to licensed professionals via onsite community visits or telehealth appointments
- Support for the many challenges life can bring, including difficult transitions, anxiety and depression, withdrawal from social activities, existing mental health concerns, loss and grief, and more
- Residents also utilize this service as an extra source of support to help them manage symptoms of cognitive decline and dementia or memory loss
Direct-to-Community Pharmacy Coordination
We work with Greater Boston Long Term Care Pharmacy to provide traditional pharmacy care and medication delivery. Now, you no longer have to worry about traveling to pick up prescriptions! We also coordinate with the pharmac so that you can access comprehensive drug reviews and assessments if you prefer.
Visiting Nurse and Speech, Physical & Occupational Therapy Resources
If you would like to receive PT, OT, and speech therapy directly in the community, we partner with Caretenders to provide rehabilitation services to help you return to your normal activities as soon as possible.
Vision, Dental, Hearing & Podiatry Providers
Other providers in our network of care carry out specialized services such as dental care, vision, and podiatry, some of which are offered onsite at our community.

- Transcare Mobile Health (Podiatry), onsite quarterly
Hospice Partnerships
We also work with Compassionate Companions, Faith and Family Hospice, and Salmon Hospice for onsite palliative, hospice, and bereavement support.
Outside Care Coordination
If your loved one needs to leave the comunity for additional care, we’ll be here to support you throughout the treatment process. We will keep in regular contact with residents, families, and the hospital or rehab center while our nursing team connects with Case Managers to plan for a smooth transition back to Cornerstone.
Our goal is to help your loved one return home to the community. We will work together to update their care plan as needed and support your loved one with any additional services they may need.
